The Transition Medicine Clinic at Baylor College of Medicine, a medical home for adults with intellectual and developmental disabilities, is opening a second clinic at the Center for Pursuit, located at 4400 Harrisburg Blvd., Houston, TX 77011. The partnership will expand the clinic and the Center’s mission to further opportunities and resources for individuals with intellectual and developmental disabilities.

The Transition Medicine Clinic helps transition individuals with intellectual and developmental disabilities into adulthood and ensures that they have lifelong access to healthcare and community resources. The location at the Center will expand access to community resources such as independent living, job training, day activities and behavioral health services. Medical residents and students from Baylor and other institutions will also have an opportunity to learn about healthcare and community resources for individuals with intellectual and developmental disabilities at the new location.

“Adults with intellectual and developmental disabilities often have difficulty finding providers who are knowledgeable about the healthcare and social supports that they need,” said Dr. Ellen Fremion, associate professor of medicine and pediatrics and the clinic director at the Center for Pursuit location. “The partnership between the Transition Medicine Clinic and Center for Pursuit is an opportunity to overcome this barrier for individuals with intellectual and developmental disabilities in the Houston community.”

“We are beyond thrilled that the Transition Medicine Clinic is opening its second location at the Center for Pursuit,” said Center President & CEO Charles C. Canton. “The Center for Pursuit advocates for the choice, growth and independence of adults with intellectual and developmental disabilities and autism by providing them with various community activities, housing and employment services. While the Center already offers a dental clinic and behavioral health services, this is the first time it will have an on-site medical partner. The new clinic location will provide the Center and the community with access to superior healthcare in a convenient setting.”

The new clinic also will accept clients from the Greater Houston area. Since the clinic is outside of the Texas Medical Center and on the Metro rail line, it provides them with greater flexibility to choose where they can receive care.

“We are hoping that this is going to be a partnership that will catapult the Center for Pursuit and the Transition Medicine Clinic into even more of the national spotlight,” said Dr. Cynthia Peacock, associate professor of medicine and pediatrics and director of the Transition Medicine Clinic at Baylor.

The Transition Medicine Clinic, originally founded in 2005, is a partnership between Texas Children’s Hospital and Baylor College of Medicine that ensures adults with intellectual and developmental disabilities have ongoing care and support after transitioning out of the pediatric environment.

“I am absolutely delighted that the Transition Medicine Clinic has opened on the new campus of the Center for Pursuit,” said David Baldwin, chair of the Board of Trustees at Baylor and also a member of the Pursuit Foundation board. “Dr. Peacock and Dr. Fremion’s groundbreaking work will add a tremendous feature to the Center’s offering and will also bring their services outside the Texas Medical Center, and into East Houston. We are very excited about the future of this joint enterprise between Baylor College of Medicine, Texas Children’s Hospital and the Center for Pursuit.”
