Use this form to request simulation events involving Procedural Skills Training (using simulation devices, including manikins, task trainers, ultrasound, etc.), Scenario-based Skills Training (using high-fidelity manikins such as SimMan), or Simulation Room Reservations (no staff support or equipment needed).
Use this form to request an event involving Standardized Patients. You will be provided with a handbook and series of templates that walk you through the process of developing a simulated patient case, including a door note, script for the SP, grading methods, checklists, debrief, etc. The case development process typically takes 4-6 months to complete.
Use this form to modify a scheduled event. Please have the original event request number available.
Use this form to request an equipment loan when no staff support or facilities are needed.
Use this form to hire Physical Exam Simulated Patients for your Ultrasound Education Event when no other staff support or facilities are needed.
Planning and implementing simulation activities involves several steps to ensure success. The following represents the optimal timeline for new simulation activities. We look forward to partnering with you!
1. Two to three months before, submit our online event request
The simulation team will initially meet to review the request, reserve the date (or offer alternatives), and assign staff. You will receive an email confirmation within a week or so.
Note: Standardized Patient cases are developed using the Faculty Blueprint for SP Cases and require four to six months' advance notice. After we receive your event request form, a member of our education team will contact you.
Note: If your activity requires a human cadaver, do not submit this form. Instead, please contact Caroline M. Kosnik with the Anatomy and Willed Body Program for further information, scheduling, and implementation.
2. Eight to nine weeks before, collaborate with the Simulation Team to plan the activity
Attend a planning meeting with the simulation core team for new events (or huddle with the operations staff for recurring events) to review details.
- Identify learning objectives.
- Select the appropriate simulation modality.
- Review the learner orientation to procedural skills lab activities.
- Define how the simulation lab will be evaluated.
3. Six to eight weeks before, create educational materials
The Simulation team will provide you with a Faculty Blueprint for Simulation Activities to develop your lab materials, including the following:
- Case Scenario
- Learner Brief
- Learner Debrief
- Learner evaluation Tool
4. Four to five weeks before, submit materials to the simulation team for review and approval
Based on the details in your educational materials, the event operations lead will create a lab summary and email it to you about four weeks before your activity.
The team will acquire equipment, models, and supplies and build the event into the learning management system or the high-fidelity manikin.
5. Four weeks before, review and approve the event summary provided by the operations team member overseeing your lab
6. Two to three weeks before, attend a pilot of the activity if necessary
7. Day of the event, co-facilitate the simulation activity
The simulation team will set up the activity according to the event summary and work with you to ensure its success. The activity survey will be available to learners via QR Code.
8. After the event, evaluate the simulation activity
Attend an activity review meeting with simulation staff.
