Transitioning from Pediatric to Adult Care
The Baylor Medicine Transition Medicine Clinic offers comprehensive healthcare, numerous wrap-around services, and social support services to help adult patients with developmental disabilities who typically struggle to find adult healthcare services.
The clinic serves the needs of young adults and adults diagnosed with Spina Bifida, Autism, Down syndrome, Cerebral Palsy and a wide range of genetic diseases.

We teach young adults important skills for navigating adulthood, train healthcare professionals and work with community organizations to reduce barriers that these young adults are often confronted with, and hold outreach events to help families and professionals understand how best to help them.
As part of an academic institution, we offer medical students, residents, nursing and social work students’ opportunities to gain valuable training to address the needs of this growing population of adult patients with developmental disabilities.
