Referral Process
Once our Referring Provider Team receives a referral request, we have streamlined the process to ensure efficient and comprehensive care coordination:
A Referring Provider Specialist will create a referral and reach out to the referring provider if there are any additional questions. For referrals submitted via EpicCare Link, online, by email, or by fax, if a confirmation of receipt has not been received within one business day, please call 713-798-7808.
A Referring Provider Specialist will contact the patient to collect further demographic and medical information, ensuring a comprehensive understanding of the patient’s needs.
Our clinical team will review the patient’s medical records. Should additional medical documentation be needed, the team will request these from either the patient or the provider’s office.
Our Insurance Verification team will verify the patient’s insurance details and obtain any necessary financial authorizations, ensuring a seamless care pathway. For more information please visit the Billing Office.
Patients will be informed of their appointment details via personal communication preference which will also include instructions for downloading and using the Baylor Medicine MyChart application for easy access to health records and appointment management. Our team will provide timely updates to the referring provider about the status of the patient’s appointment referral, ensuring transparent and continuous communication throughout the process.
