Online — Fill out the online form below. Please allow for a two business day turnaround for requests received Monday through Friday. Requests received on weekends or holidays will receive a response two business days after the start of business the following week.
Phone — For a list of specialties and their associated phone numbers please view our Clinical Services Directory. For general inquiries call 713-798-1000 between 8 a.m. and 5 p.m., Monday through Friday.
Note: All new patients are required to provide insurance at the time of scheduling.
