Transitions of care should be standardized to ensure continuity, coordination and safety in patient care. Accordingly, we will:
- Designate I-PASS as the institutional standardized care transitions tool;
- Require clinical training programs to implement I-PASS as the standardized care transitions tool among housestaff;
- Train faculty to observe I-PASS transitions of care among housestaff.
