Internal Medicine/Medical Genetics Rotations and Call Schedule

Typically, rotations are in blocks of four weeks. Combined internal medicine/medical genetics residents spend the first year as interns in internal medicine. In years two through four, the combined internal medicine/medical genetics residents have six to seven blocks scheduled in genetics with the remaining blocks scheduled in medicine.

Call Schedule

Two residents are assigned to the inpatient genetics consult service rotation, and they share call from home while on this rotation. There are no home or in-house calls on other genetics rotations.  Internal medicine rotations are on a 4+4 schedule where residents alternate between wards/ICU rotations and specialty/outpatient rotations. Residents have three weeks of vacation each year. The following is a representative resident schedule.

Rotations by Year

PGY-1

Internal Medicine Rotations (13 blocks)

A combination of rotations in the inpatient setting (Ben Taub, Baylor-St. Luke’s Hospital, MD Anderson, and VA), intensive care units (Ben Taub), ambulatory clinics, and emergency medicine. There are:

  • five inpatient ward blocks
  • three subspecialty/ambulatory blocks
  • two night-float blocks
  • one block each of MICU, CCU and ER.

Rotations are on a 4+4 schedule where residents alternate between wards/ICU rotations and specialty/outpatient rotations.

PGY-2

Internal Medicine Rotations (7 blocks)

A combination of rotations that includes:

  • two ambulatory blocks
  • one ward supervisor block
  • two elective block (shared with genetics)
  • one upper level IM block
  • one neurology block

Residents alternate between wards/ICU rotations and specialty/outpatient rotations.

Genetics Inpatient Consult Service (2 blocks)

Participate in the management of inborn errors of metabolism for patients admitted to Texas Children’s Hospital or Baylor-St. Luke’s Hospital

Provide inpatient genetics consultations for children and adults at Texas Children’s Hospital and Pavilion for Women, Baylor-St. Luke’s Hospital, Ben Taub Hospital and Texas Woman’s Hospital

Work with nurses, genetic counselors and medical biochemical genetics fellows to assist physicians, parents and newborn screening nurses with patient management concerns

Metabolic/Skeletal Dysplasia – Texas Children’s Hospital and Baylor Adult Clinic (2 blocks)

Participate in weekly outpatient diagnostic evaluations and chronic management of children and adults with inborn errors of metabolism

Participate in monthly outpatient diagnostic evaluations and chronic management of individuals with skeletal dysplasias

Participate in weekly outpatient diagnostic evaluations in the pediatric genetics clinic

Adult Genetics - Harris Health, VAMC & Baylor Clinic (2 blocks)

Participate in outpatient diagnostic evaluations and chronic management of adults with personal or family cancer risk, connective tissue disorders, cardiac disorders and others referred for genetic evaluations

PGY-3

Internal Medicine Rotations (5 blocks)

A combination of rotations that includes:

  • two ambulatory blocks
  • one VA ward supervisor block
  • one MICU supervisor block
  • one elective block (shared with genetics)

Residents alternate between wards/ICU rotations and specialty/outpatient rotations.

Genetics Inpatient Consult Service (2 blocks)

Participate in the management of inborn errors of metabolism for patients admitted to Texas Children’s Hospital or Baylor-St. Luke’s Hospital

Provide inpatient genetics consultations for children and adults at Texas Children’s Hospital and Pavilion for Women, Baylor-St. Luke’s Hospital, Ben Taub Hospital and Texas Woman’s Hospital

Work with nurses, genetic counselors and medical biochemical genetics fellows to assist physicians, parents and newborn screening nurses with patient management concerns

Prenatal Genetics – Texas Children’s Pavilion for Women (1 block)

Participate in outpatient prenatal consultations

Diagnostic Laboratory – Baylor Genetics Laboratory (2 blocks)

Observe laboratory techniques and interpretation of laboratory results

Neurogenetics – Texas Children’s Hospital (1 block)

Participate in outpatient diagnostic evaluations and chronic management of neurogenetic disorders and muscular dystrophy

Hematology/Oncology (1 blocks)

Participate in the outpatient management of cancer genetic syndromes and bone marrow failure disorders

Pediatrics Genetics Outpatient Clinic - Texas Children’s Hospital (1 block)

Experience in outpatient general pediatric clinics, focusing on the diagnosis and management of genetic disorders.

PGY-4

Internal Medicine Rotations (7 blocks)

A combination of rotations that includes:

  • three elective blocks
  • two Ben Taub ward supervisor blocks
  • one CCU supervisor block 
  • one ambulatory block
  • one general medicine consult block.

Residents alternate between wards/ICU rotations and specialty/outpatient rotations.

Genetics Inpatient Consult Service (2 blocks)

Participate in the management of inborn errors of metabolism for patients admitted to Texas Children’s Hospital or Baylor-St. Luke’s Hospital

Provide inpatient genetics consultations for children and adults at Texas Children’s Hospital and Pavilion for Women, Baylor-St. Luke’s Hospital, Ben Taub Hospital and Texas Woman’s Hospital

Work with nurses, genetic counselors and medical biochemical genetics fellows to assist physicians, parents and newborn screening nurses with patient management concerns

Adult Genetics - Harris Health, VAMC & Baylor Clinic (2 blocks)

Participate in outpatient diagnostic evaluations and chronic management of adults with personal or family cancer risk, connective tissue disorders, cardiac disorders and others referred for genetic evaluations

TeleHealth – VAMC and Consultagene (1 block)

Provide telehealth services for the VA region and through the online portal Consultagene

Pediatrics Genetics Outpatient Clinic - Texas Children’s Hospital (1 block)

Experience in outpatient general pediatric clinics, focusing on the diagnosis and management of genetic disorders.
