Pediatric Hospital Medicine Curriculum

Curriculum

Through longitudinal experiences in clinical practice, teaching/education, clinical research, quality improvement/ patient safety, and administrative/ leadership/ career development Pediatric Hospital Medicine Fellowship trains fellows to:

  • Demonstrate proficiency in the clinical practice of pediatric hospital medicine.
  • Professionally interact in the education of other health care providers, patients, and families.
  • Develop a career niche in one/more of the following domains: teaching/ education, clinical research, quality improvement/ patient safety, advocacy, and administrative/ leadership.
  • Lead in the field of pediatric hospital medicine in an academic and/or community-based hospital.

  • 24 weeks of PHM at Main Campus [to include 1 dedicated week of procedural sedation in addition to full spectrum of general pediatric inpatient medicine (content of which includes care of newborns, care of patients with complex chronic diseases, care of patients with surgical problems, and care of patients receiving palliative care)]
  • 2 weeks of Care of the Newborn at Texas Children’s Main Campus
  • 2 weeks of Care of the Medically Complex Child (inpatient) at Texas Children’s Main Campus
  • 4 weeks of Community Medicine at Texas Children’s West Campus

  • 2 weeks- Introduction to PHM Individualized Curriculum
  • 6 weeks- Scholarly Work Introduction in first year of fellowship
  • 4 weeks- Advanced Scholarly Work in second year of fellowship
  • 20 weeks- Additional Individualized Curriculum Experiences
    • The electives below are encouraged for consideration to meet the fellow’s clinical and academic learning needs and career plans, with a suggested 8 weeks minimum of additional clinical experience over 1st and 2nd year of fellowship (~4 weeks per year) from the following list.

      • Pain Management Team with emphasis on addiction
      • Advanced Pediatric Palliative Care
      • Physical Medicine and Rehabilitation (PM&R)
      • Additional Well Newborn Care
      • Outpatient Complex Care Clinic

Dedicated to research curriculum and scholarly activity, including the development of requisite skills, project completion, written work product, and presentation of results to the Scholarship Oversight Committee (SOC).

Year 1

Core PHM skills - 18 weeks

  • TCH Main Campus - 11 weeks
  • Care of the Medically Complex Child (inpatient) - 2 weeks
  • Sedation  - 1 week
  • Community Medicine at TCH West Campus in Katy, TX - 4 weeks

Individualized Curriculum 16 weeks

  • Intro to Individualized Curriculum - 2 weeks
  • Scholarly Work Introduction - 6 weeks
  • Elective time - 8 weeks

Scholarly Activity - 14 weeks

Year 2

Core PHM skills - 14 weeks

  • TCH Main Campus - 12 weeks
  • Care of the Newborn - 2 weeks

Didactic

Our section has a robust educational conference curriculum with a faculty champion(s) and fellow representative(s) for each lecture series. Fellow representatives work with faculty champions to review conference content, structure, educational objectives and future directions. Fellows have a significant role in advising the curriculum committee to ensure conferences are meeting the educational needs of the fellows and reviewing the PHM board content specifications.

Fellows College is an innovative and collaborative approach to address the ACGME nonclinical skills required for subspecialty training. It was implemented at Baylor College of Medicine in 2011. The program has expanded from approximately 11 sessions in the first year to now about two sessions per month.

Scholarly Activity

Fellows have 32 weeks of dedicated time for scholarly activity/research. This includes:

  • Completion of Collaborative Institutional Training Initiative (CITI)
  • Submission of an Institutional Review Board (IRB) protocol for a research project
  • Participation in PHM Advisory Committee on Excellence in Research (ACER)/Works in Progress sessions
  • Participation in Fellows College Topics in Research sessions
  • Participation in PHM Fellows Writing Series
  • Participation in Scholarship Oversight Committee
  • Submission of an APA/AAP/SHM abstract submission (and presentation if accepted)
  • Attendance at one academic meeting per year
  • Presentation of research at Scholarly Oversight Committee Meeting and at TCH

  • Completion of a research project mentored by an experienced faculty member (See separate document for mentor requirements.)
  • In accordance with American Board of Pediatrics Subspecialty Requirements:
    • In addition to participating in a core curriculum in scholarly activities, all fellows will be expected to engage in projects in which they develop hypotheses or in projects of substantive scholarly exploration and analysis that require critical thinking.
    • Areas in which scholarly activity may be pursued include, but are not limited to:  basic, clinical, or translational biomedicine; health services; quality improvement; bioethics; education; and public policy. In addition to biomedical research, examples of acceptable activities might include a critical meta-analysis of the literature, a systematic review of clinical practice, a critical analysis of public policy, or a curriculum development project with an assessment component. Involvement in scholarly activities must result in the generation of a specific written “work product.”
    • Work Product is a peer-reviewed publication in which a fellow played a substantial role and examples include the following:
      • An in-depth manuscript describing a completed project
      • A thesis or dissertation written in connection with the pursuit of an advanced degree
      • An extramural grant application that has either been accepted or favorably reviewed
      • A progress report for projects of exceptional complexity, such as a multi-year clinical trial
    • Review of scholarly activity and the written work product will occur at the local level, with each fellow having a Scholarship Oversight Committee responsible for overseeing and assessing the progress of each fellow and verifying to the ABP that the requirement has been met.
