The Baylor College of Medicine Transition Medicine section is nationally recognized for its excellence in healthcare and education. It was developed as a joint project with Texas Children’s Hospital as a way to address the needs of patients with chronic disease or disability who are transitioning into adulthood.
Many physicians from the section are national experts in their field. Together, they provide an array of services, from primary care for those with complex, chronic conditions to cystic fibrosis care. The faculty here take care of patients with some of the most rare and complex conditions.
