The first step we require when scheduling a new appointment is current office notes and history and physical be faxed to our office from your referring physician for our physician's review.

Prescription Refills

The pharmacist must call in your request for a new prescription or refills. Please allow a 24-hour turnaround time. Only controlled substance request/refills may be called in directly to the prescription line.
