Transition Medicine is a unique Section within the Department of Medicine. The team provides healthcare and social support services to young adults and adults with developmental disabilities who have been receiving care from Texas Children's Hospital and their affiliates. Many of the young adults/adults seen in our clinic have conditions such as Down syndrome, Spina Bifida, Autism, Cerebral Palsy, and other IDDs.

The TMC’s specially trained physicians and licensed medical social workers help these patients and their family access adult primary care, navigate the healthcare system, and identify and maintain the social services essential to their continued well-being. Our team creates long-term care plans for success and address current issues that adults with developmental disabilities face.
