By working closely with patients, their families and their providers, specialists at the clinic provide information on and help with important topics such as accessing medical and social services, navigating insurance issues and overcoming barriers.

The Section of Transition Medicine provides elective rotations for medical students and residents in primary care at Baylor College of Medicine. In addition, Transition Medicine partners with three outside institutions to provide an internship placement for social work students and a learning rotation for nursing students.

The Section of Transition Medicine trains healthcare professionals and organizations around the community with whom young adults with chronic issues might interact, so as to help reduce barriers to their success as an adult..

The section also holds regular outreach events such as the free Pathways to Adulthood seminar, which helps families and professionals better plan for a young adult’s life after high school.
