The Section of Transition Medicine addresses the health care needs of older adolescents with developmental disabilities who will require significant care coordination and case management in addition to ongoing medical care as they enter the adult health care system. This includes Autism, Cerebral Palsy, Spina Bifida, Down syndrome and other genetic disorders for which pediatric care is no longer available.
