If you are interested in establishing care for yourself or for someone under your care with the Transition Medicine Clinic, please complete our New Patient Intake Questionnaire available in MyChart. To start this process, call the clinic at (713) 798-6333. Our call staff will help you set up your MyChart record and log in. Once that is done, or if you already are enrolled with MyChart, you will need to call the clinic again, and we will send the questionnaire to you. Our medical director will review the case and determine if we will be the best outlet of care.
