Register in Epic
All new patients to the Baylor College of Medicine clinics must register with Baylor College of Medicine.

The scheduling team will ask you to provide basic personal information and details regarding your diagnosis and insurance or other payment plans.
Please activate MyChart. A code is needed to activate MyChart. If you have not received a MyChart activation code you can go to mychart.bcm.edu, select the Sign Up Now button and then the Request Online button.
MyChart offers patients personalized and secure online access to portions of their medical records. It enables you to securely use the Internet to help manage and receive information about your health.
Complete Intake Paperwork
All patients must complete the Adult Genetics Intake forms before they can be scheduled for an appointment. This allows us to schedule your appointment with the most appropriate physician. It also may help the providers prepare for your visit.
Please complete the General Intake form, the Family History form, the Health Care Providers form, and the Patient Question List. Please send the requested records, if available.
If you are coming for evaluation for any of the following: chronic pain, joint hypermobility, Ehlers-Danlos syndrome [EDS], dysautonomia, dizziness, syncope [fainting], joint dislocations or other connective tissue disorder, also please complete the Connective Tissue Disorder Intake form. Not all individuals attempting to schedule an appointment will need to complete this form.
Based on your responses on the intake forms, medical records may be needed to ensure that you receive optimal care. You can speed up the scheduling process by sending the results of any prior genetic testing (your own or a family member’s) or including the reports wherever indicated on the intake form. Otherwise, you will be informed by the clinic if you need to provide any additional records prior to scheduling.

We take our patients' privacy very seriously. Please complete the following forms specific to releasing information.
Please complete ONLY Section B and C of the Alternative Communications and Designated Contacts form.
If a family member contacts us for information regarding your genetic diagnosis/medical care, and they are not listed on this form we will be unable to discuss your information with them until you provide permission.
Please complete the Authorization for Release of Protected Health Information form.
Some of our patients are under the care of a parent/guardian. If there is a guardian or person with medical power of attorney for the patient, please send documentation of guardianship or medical power of attorney.
How to Send the Forms
We have three methods you can use to submit your completed forms:

Upon review of your materials you will be contacted by the Genetics Clinic. Also note that in an effort to be serve all patients, we are unable to offer an appointment to individuals who have not submitted all forms and signed up for MyChart.
At the Appointment
- Upon arrival, please check in with the front desk. You will be asked for your photo identification and insurance card.
- Please plan to arrive about 15 minutes prior to your scheduled appointment.
- For the convenience of all our patients, individuals who arrive 20 minutes or more late will need to reschedule. This helps to keep our clinic running on time.
- Generally, patients do not need to fast before their appointment. While genetic testing, when appropriate, is often ordered the day of the appointment, genetic testing does not require fasting.
Billing Information for Prospective Patients
For questions about billing for the appointment, please contact Baylor College of Medicine’s Billing Office at (713) 798-1900.
During your visit, we may determine if genetic testing is needed based on review of your medical history and physical examination findings. We cannot say before your appointment which test, if any, will be ordered. Questions about insurance coverage of genetic tests are best answered by the laboratory performing the testing and by your insurance provider (once the appropriate test is determined). Upon request at your visit, we can provide you with contact information for the laboratory that will perform your test.
