Vaccines for family & friends

COVID-19 vaccines are now available through Baylor to the family and friends of BCM employees, students and trainees. To schedule, have your family member or friend complete this form. They will receive an email from BCM MyChart to select a date and time for their vaccination. When they receive their confirmation email for their appointment, your family or friend is strongly encouraged to set up a BCM MyChart account if they don't already have one – this will make it easier and more efficient to check in and schedule their second dose.
