Test Requisition: Tumor Analysis
Specimen Type: Bone Marrow
Requirements: Draw bone marrow in an EDTA (purple-top) tube(s) and send 3 mL (Min: 1 mL).

Shipping Conditions: Ship at refrigerated temperature in an insulated container by overnight courier. Specimen should arrive in the laboratory within 24 hrs of collection. Do not heat or freeze.

Unacceptable Conditions: Frozen specimens. Clotted or grossly hemolyzed specimens.

Stability: Ambient: 24 hours; Refrigerated: 5 days; Frozen: Unacceptable

Specimen Type: FFPE- Tumor Tissue
Requirements: Formalin fix (10 percent neutral buffered formalin) and paraffin embed tissue. Protect from excessive heat. Please include a copy of corresponding surgical pathology report.

Shipping Conditions: Ship at ambient temperature (18-25C/64-77F) or refrigerated temperature in an insulated container by overnight courier. Ship in cooled container during summer months. Do not heat or freeze.

Unacceptable Conditions: Specimens fixed/processed in alternative fixatives or heavy metal fixatives (B-4 or B-5) or tissue sections on slides. Decalcified specimens.

Stability: Ambient: Indefinitely; Refrigerated: Indefinitely; Frozen: Unacceptable

Specimen Type: Fresh Frozen Tissue
Requirements: 100 mg (0.5-2.0 cm3) fresh tissue snap frozen at -20C. Please include a copy of corresponding surgical pathology report.

Shipping Conditions: Ship frozen on dry ice in an insulated container by overnight courier. Specimen should arrive in the laboratory within 24 hrs of shipment.

Stability: Ambient: Unacceptable; Refrigerated: 2 hours; Frozen: 1 year

Requirements: Draw whole blood in an EDTA (purple-top) tube(s) and send 5 mL (Min: 1 mL).
