Specimen Type: FFPE - Slide(s)
Requirements: 5 unstained 5um FFPE slides (Min: 3 slides) containing adequate amounts of tumor to be analyzed. Please include a copy of corresponding surgical pathology report.
Shipping Conditions: Ship at ambient temperature in an insulated container by overnight courier. If shipping during the summer months please include a cold-pack to avoid extreme temperatures. Do not heat or freeze.

Unacceptable Conditions: Frozen specimens. Specimens fixed/processed in alternative fixatives (alcohol, PreferŪ) or heavy metal fixatives (B-4 or B-5). Decalcified specimens.

Stability: Ambient: Indefinitely; Refrigerated: Indefinitely; Frozen: Unacceptable

Requirements: Formalin fix (10 percent neutral buffered formalin) and paraffin embed tissue. Protect from excessive heat. Please include a copy of corresponding surgical pathology report. Tissue block will be returned after testing.
