Special Notes: Test code 8405 is to be used when requesting FISH of a known familial copy number variant(s) for which the BG does not provide a separate specific test code. This should only be used when the BG has already identified the copy number variant in the proband/original patient by chromosome microarray analysis or whole exome sequencing. If this test is not offered in the proband's report or proband testing was not originally performed at this laboratory, please call to discuss prior to sending sample.
