To request an appointment for the health fair:
- Download and print out the forms listed below.
- Complete all forms (except the HIPAA privacy notice, which you should read and keep for your records).
- Scan/e-mail completed forms to marfan2010@bcm.edu or fax (832-355-9940) or mail to the following address:
