ADVANCED REGISTRATION IS REQUIRED
The LINC Course registration fee includes admission to the general
session, access to exhibits, continental breakfasts, breaks, lunch, social
events, and admission to The Technology Fair. Participants can also
register for The Hands-On Experience only. This component of the
course requires a separate registration, and is open only to physicians. Space is limited.
Participants also can register for both the LINC Course and The
Hands-On Experience at a discounted combination registration
fee. Your registration will not be processed until payment is received.
Your registration will be confirmed. If you have not received
confirmation at least five business days before the course start date,
contact the registration office immediately (281-858-7779), Shellie@stonewaterproductions.com.
If you plan to register on-site, contact the registration office at least
24 hours in advance to confirm space availability. The course reserves
the right to limit the number of participants and will not be responsible
for any expenses incurred by an individual whose registration is not
confirmed and for whom space is not available.
You can register online at www.linccourse.org. Registration fees
must be paid in U.S. dollars. American Express, MasterCard and Visa
are accepted.
CANCELLATION/REFUND POLICY
Requests for registration refunds must be in writing and received
by the registration office at least 14 business days before the course
begins. The date the request is received by the registration office will
be considered the cancellation date. Requests received after the refund
deadline will not be processed. Cancellations are subject to a $100
administrative fee deducted from the registration fee paid to cover
guarantees and other expenses. Requests should be faxed to 832-550-
3400 or emailed to shellie@stonewaterproductions.com.
The LINC Course Houston reserves the right to cancel activities,
not less than 10 business days before the scheduled date, if extenuating
circumstances make it necessary. Registrants will be notified at the
contact number and email indicated on their registration form, followed
by written notification. If an activity is cancelled, LINC Course
Houston’s liability is limited to the registration fee paid. NOTE: If
payment is made by check, a social security number is required to
process the refund.

A $20.00 faxed/mail fee will be added to your registration
should you wish to register by fax or mail.
Please note you can register online and pay by check by simply clicking on the “pay by check” box on the online form. Payment by check following online registration must be received
within 14 days of registration. You will not be charged the additional $20.00 fee if you register online and forward the check in the mail.
