Normal Obstetrics
To understand the changes accompanying pregnancy, including the anatomical and physiological changes of early pregnancy and the endocrinological changes that occur in mother and fetus as pregnancy progresses; to learn to use patient records and evaluations of patient reactions to gain competency in providing comprehensive prenatal care; to know the actions and effects of various drugs, analgesics and anesthetic agents on both the mother and the developing fetus; to perform a normal spontaneous vaginal delivery or operative vaginal and abdominal delivery; to be able to evaluate a postpartum patient and be familiar with the physiological and psychological changes, including lactation, that occur during the puerperium.
High-Risk and Operative Obstetrics
To manage abortion, signs and symptoms of ectopic pregnancy and causes of third-trimester bleeding; to become proficient in developing a management plan for medical and surgical conditions complicating pregnancy, including pregnancy-induced hypertension, Rh Disease, intrauterine growth retardation, multiple pregnancy and premature rupture of membranes; to learn the causes of premature labor and the effects of various tocolytic agents in its treatment; to be knowledgeable of the pathophysiology of dystocia and capable of differential diagnosis and informed management for each cause; to be competent in the use of ultrasound equipment; to be familiar with operative techniques for amniocentesis, forceps rotation, breech delivery, episiotomy, third- and fourth-degree perineal repairs, vaginal wall and cervical laceration repairs, version and extraction, hypogastric artery ligation and peripartum hysterectomy; to recognize and manage various causes of postpartum hemorrhage, obstetrical shock and postpartum fever; to define and calculate maternal and perinatal mortality and morbidity rates; to gain knowledge of genetic diseases requiring special counseling with patients.
Ultrasonography
To gain a basic understanding of the role of ultrasound in obstetrics; to integrate the ultrasound examination into patient management procedures; to interpret gynecologic ultrasound images, identifying pelvic tumors, ectopic pregnancies and other pelvic pathology.
Family Planning / Psychosexual Counseling
To become thoroughly familiar with the physiology and pharmacology of contraception; to become adept at sexual counseling; to learn to diagnose, treat and prevent sexually-transmitted diseases; to prevent and treat complications of abortion.
Operative Gynecology
To gain a thorough knowledge of the female reproductive tract, its relationship to surrounding organs, blood supply, nerve supply and the lymphatic drainage of genital organs; to become expert in the performance of routine vaginal operative procedures, including vaginal hysterectomy, anterior colporrhaphy, colpoperineorrhaphy, repair of enterocele, vaginal sacrospinalis ligament suspension, cervical conization, colpotomy, repair of vesicovaginal and rectovaginal fistula, D&C and pregnancy termination; to become familiar with abdominal hysterectomy, anterior urethropexy and vaginal sacropexy procedures; to become proficient in performing conservative surgery for endometriosis and other procedures in restoring fertility to an infertile woman; to learn radical operative procedures for pelvic malignancies, such as radical hysterectomy and pelvic lymphadenectomy; to be prepared to perform operative procedures involving the vulva, including vulvectomy, skinning vulvectomy and local excision of various vulvar lesions, such as carcinoma in situ, marsupialization of Bartholin duct cyst and excision of Bartholin gland; to manage operative complications, including bladder injury, intestinal lacerations, post-operative ureteral injury, pelvic abscess and intestinal obstruction; to become proficient in performing gynecologic endoscopic procedures, such as diagnostic and therapeutic laparoscopy (including laparoscopic sterilization) and diagnostic and operative hysteroscopy; to develop sufficient knowledge and technical skill to learn and perform new operative procedures as they are proved beneficial and safe for patients post-training.
Pathology
To become knowledgeable about the normal histology of the female reproductive tract, cyclic changes of the ovaries and endometrium and to be able to "date" the endometrium accurately; to be skilled in identification of both gross and microscopic surgical pathology; to be familiar with various histochemical and immunological staining techniques; to be familiar with frozen section diagnosis.
Gynecologic Oncology
To effectively manage the general behavior, concepts of treatment and treatment programs for gynecologic cancer; to learn to diagnose and treat gynecologic cancer in its early stages and to manage more advanced diseases, making logical decisions about referrals to gynecologic oncologists; to learn the techniques of colposcopy, as well as procedures for out-patient treatment of CIN.
Reproductive Endocrinology
After extensive study of the function of the hypothalamic-pituitary- ovarian axis, to be able to diagnose and treat such problems as anovulation (secondary to stress, immature HPO-axis, hyperandrogenism, hyperprolactinemia, drugs, pituitary tumors and ovarian dysgenesis), abnormal sexual development, menopause (both surgical and spontaneous) and metabolic disorders related to reproduction; to be well-informed about the pathogenesis and medical treatment of endometriosis, pelvic inflammatory disease and infertility; to develop the surgical skills necessary to properly diagnose and treat iatrogenic and non-iatrogenic tubal occlusion, pelvic adhesive disease, endometriosis, uterine pathology (such as Mullerian anomalies, intrauterine synechiae and uterine leio- myomata) and vaginal agenesis; to develop expertise in performance and interpretation of hysterosalpingography and laparoscopy and the use of the operating microscope and the laser.
Breast Disease
After learning to include a properly done breast examination in each routine gynecological exam and to retain accurately documented patient records of significant physical findings, to also learn to educate, advise and counsel patients and their families about breast disease; to develop working relationships with appropriate professional colleagues to provide an integrated "team approach" to immediate and long-term care of women with breast cancer; to learn the embryology, anatomy, growth and development, physiology, endocrinology and pathology of the breast necessary to care for patients who experience lactation problems or who develop noncancerous breast diseases.
Genetics
To have thorough knowledge of the indications for genetic amniocentesis, fetal blood sampling and chorionic villus sampling; to thoroughly understand the risks, benefits and limitations of these tests.
Infectious Diseases
To develop a basic understanding of the relationship of microorganisms and disease processes in the human host; to recognize characteristics of specific bacteria; to understand different classes of antibiotics and their actions in the host against bacteria; to understand postoperative care and infectious processes; to be able to recognize precursors of infection in an obstetrical and gynecological patient; to know how to determine risk factors for patients.
