We have found that the provider directories, issued by the insurance companies, do not always accurately represent our participating physicians. The following list provides the names of the plans and products for which we are contracted. If you do not find a name on this list, please call our patient services staff. We will help you determine whether we are providers for your specific coverage. Some insurance plans provide infertility coverage, but the benefits vary widely. Please call your customer service phone number on your insurance card to get the details on your particular infertility benefits. The Department of Ob/Gyn is contracted for most insurance plans. Our staff will call to verify your benefits. Please have your ID card available when you call for your appointment. Important Notice to Patients: Some insurance companies, most recently HMO Blue of Texas, now require members with HMO coverage to choose a Primary Care Physician (PCP) in a Limited Provider Network, such as Kelsey-Seybold or Renaissance IPA. If you have a PCP in a Limited Provider Network, you will need a referral in order to see a Baylor Ob/Gyn physician. Please ask your Primary Care Physician to process a referral for you or ask your PCP to call our patient services department. Having the required referral in place prior to your visit will provide for prompt and correct payment of the insurance benefits to which you are entitled.
