Before your appointment,
- Please contact your primary physician’s nurse or secretary if you need help deciding which studies you need to bring
- Please phone the day before your appointment to confirm
- Also, please be certain that we have your contact information
The day of your appointment, please
- Bring your insurance information
- Arrive 20 minutes early to complete registration forms
- Bring X-rays, CT scans or MRI scans taken for your current medical condition, if available
