Today’s visit:
• Review of the chart indicates the patient has come
  in due to the following symptoms:
  o Flu-like symptoms (fatigue, weakness,
     loss of appetite) over extended period
  o Weight loss
  o Frequent urination, especially at night
  o Blurred vision
  o Stiffness and swelling in the knees and hips
  o Abdominal pain
