Location
We are located in the Texas Medical Center, in the Fondren/Brown Building of the Methodist Hospital, Suite B160 (1st floor, next to the Fondren/Brown Elevators; the sign reads Center for Cardiovascular Disease Prevention). Convenient parking garages are nearby and valet parking is available at the Methodist Hospital entrance at 6565 Fannin Street.
Appointments
The Clinic meets by appointment. Please call 713-798-5800. Physician referrals are necessary.
There are many causes of high cholesterol or triglycerides, and it is essential that we understand your case as thoroughly as possible. Please bring the following with you to your appointment:
- Filled out questionaire that was sent to you prior to your appointment
- Copies of your medical records and labs from your referring physician
- Insurance card
- List of medications and dosages that you are taking
Also, please be advised that we do NOT take credit/debit cards. Please prepare to pay by check or cash in the exact amount of your office co-pay.
