About the Training

Nursing home transition services may be complex and sometimes perplexing, but there is both a tried and true science and an art to providing them. Here is an opportunity to learn from some of the most experienced and effective CIL experts in the country. They will give you valuable tips, strategies and insights for developing a relationship with nursing facilities, assisting individuals to identify their needs, using checklists and tools for planning, maximizing the success of moving day, providing advocacy after the move, and much more.

  • Use the ADA and the Olmstead Decision to support nusing facility transition
  • Locate and assist individuals in assessing their needs and preferences for community-based living
  • Assist individuals in developing and implementing a transition plan
  • Conduct effective post-transition activities to support individuals in maintaining their lives in the community

Target Audience:

Managers and front line workers new to transition, more experienced staff who would benefit from interactive problem-solving sessions, and participants in the ABCs of Nursing Home Transition webinars who want a more comprehensive and interactive experience.

About the Presenters

Bruce E. Darling, Executive Director, Rochester Center for Disability Rights

Bruce is co-founder and Executive Director of the Center for Disability Rights (CDR), a Rochester-based disability rights organization and Independent Living Center. During the Summer of 2000, without any new funding, but with a great deal of conviction, CDR began a project to transition people out of nursing homes and back into the community. Since that time, the project has helped hundreds of people return to community living, and CDR has hired staff dedicated to this transition work.

Jane Schiele, Director, Satellite Offices, Disability Center for Independent Living

Jane has worked at Independent Living Centers since 2000. She was the Satewide Coordinator for the grant-funded Nursing Facilities Transition Project in Colorado from 2002 to 2004. The project was so successful that it is now permanently in Colorado's legislation and is named the Community Transition Services. Currently, Jane works as the Director of Satiellite Offices for the Disability Center for Independent Living in Denver, Colorado.
