Application Procedure for Post-Master's DNP Program
Applicants meeting the admissions requirements are invited to apply.
Submit the application on or before July 1 of the year prior to which admission is being sought. Early application is encouraged. The online application is only available Feb. 15 through July 1 each year.
- Submit all required supporting documents and application fee such that they are received by the Graduate Program in Nurse Anesthesia Program Office no later than July 15 of the year prior to which admission is being sought. Applicants applying for advanced standing status with regard to the anesthesia core courses must so indicate on the application form.
- Supporting documents to be submitted by the applicant to the GPNA Office:
- Photocopy of current, unencumbered R.N. license. Applicants from states that prohibit photocopying of nursing licenses must have a letter sent from the state board of nursing directly to the GPNA Office verifying that he/she holds a current, unencumbered license to practice professional nursing.
- Photocopy of current, unencumbered APRN licensure, or other authorization to practice as a nurse anesthetist, in state of residence.
- Proof of current certification or recertification by the National Board on Certification and Recertification of Nurse Anesthetists.
- Current Curriculum Vitae.
- Application fee of $90 (submitted via PayPal).
- Mail all supporting documents to the GPNA Office at: Baylor College of Medicine
- Supporting documents that must be submitted directly from the sender (not the applicant) to the GPNA Office:
- Transcripts: Forward official transcripts from all colleges, universities and nursing schools attended.
- Evaluations from nursing and medical professionals: Provide two evaluations from healthcare professionals who are able to provide an assessment of professional performance, clinical acumen, and essential character traits. Evaluations from family members or personal friends will not be considered.
- A completed evaluation form must be submitted by each of the following:
- Current anesthesia supervisor who can attest to the applicant's professional and clinical acumen. The evaluation must be submitted on the CRNA Supervisor form.
- CRNA or physician of your choosing.
- The evaluation forms, specific to each category of evaluator, are found on our web site. From our home page, select "Admission," then "Application Forms" to print out the necessary copies.
- Please be sure to complete section one of each form before giving the form to your evaluators.

Personal or telephone interview with members of the Admissions Committee will be offered to qualified applicants. Qualified applicants will be notified of their interview date. Interviews typically are held the last Friday and Saturday in August.
