Incoming Resident or Fellow

Procedures & Instructions For Incoming House Staff

Congratulations on your provisional acceptance into the graduate medical education training programs at Baylor College of Medicine. The following are requirements for employment and appointment as house staff members.

We will cover many items at check-in, but some things have been placed on the Web to be completed prior to orientation to make the process faster and easier.


Other Documents Baylor will require from you are:

  • ACLS/ATLS/PALS Certification
  • Curriculum Vitae – Current
  • 2 Professional Reference Letters – Current
  • Original Medical School Transcript
  • 3 True Copies (Notarized) of Medical School Diploma (Dean's Verification of Pending Graduation acceptable only for those anticipating graduation. Dean's verification must be original document, copies are NOT acceptable)
  • True Copy of Prior Residency / Fellowship Training Diploma (Notarized)
  • Copy of FULL TEXAS LICENSE - Proof of DEA and DPS Registration (if applicable – Do not need to complete the Texas Standardized Credentialing Application)
  • True Copy (Notarized) of ECFMG Certificate - REQUIRED for foreign graduates (outside US or Canada)


In addition to the GME forms, Baylor’s HR department would also like to inform you that when you come in you will have to fill out their:


You must have with you two of the following original documents

  • U.S. Passport
  • U.S. Social Security Card
  • U.S. or Canadian photo driver’s license
  • U.S. certificate of citizenship
  • U.S. certificate of naturalization
  • Unexpired foreign passport
  • Permanent resident card
  • Alien registration receipt card
