The determination of free and total carnitine is useful in the diagnosis of primary disorders of carnitine metabolism and uptake as well as in organic acidopathies. In addition, patients receiving carnitine as part of a therapeutic regimen can be monitored. Carnitine analyses are performed on the tandem mass spectrometer. Both free and total carnitine values are reported and the free/total ratio is also provided.

Specimen Requirements:

Draw blood in a heparinized (green top) tube. Spin down and send 0.5 to 1 ml frozen in an appropriate vial.

Normal Range:

Free: 37 ± 8 µM
Acyl: 9.4 ± 4.5 µM
Total: 46 ± 10 µM
Free/Total: 70-94%
