Identifiers and Chief Complaint:
66 year old man with weakness and lightheadedness
 
History of the Present Illness:
He had been taking naproxen (6 tablets daily) for shoulder pain for two weeks. Three days ago he began having 3 to 4 melenic stools per day. Two days ago he developed weakness and became lightheaded. In the emergency room, he was found to be pale and a nasogastric lavage revealed coffee-ground material which cleared after 1.2 liters of levage.  
 
Previous Medical Diagnoses, Hospitalizations, and Operations:
Parkinson's Disease
Hypothyroidism
Hypertension 
Diabetes Mellitus, Type 2 
 
Medications:
Acetaminophen
Naproxen
Gabapentin
Levothyroxine
 
Personal and Social History:
Quit tobacco and alcohol use 35 years ago, previous heavy ingestion of beer for 18 years; no substance abuse; no travel; stable family and finances
 
Family History:
No relevant family history of serious diseases
