Identifiers and Chief Complaint:
45 year old man who is referred for colonoscopy  
 
History of the Present Illness:
The patient had sigmoid polyps discovered on screening flexible sigmoidoscopy.  He denies hematochezia, melena, abdominal pain, change in bowel habits or appetite, and weight loss. Informed consent was obtained by the colonoscopist just prior to the procedure. Risks, benefits, and alternatives were discussed in a 3 minute conversation witnessed by the nurse.

Coronary Artery Disease 1998 
Hyperlipidemia 1998
           
Medications:
Isosorbide mononitrate
Fosinopril
Metoprolol 
Simvastatin
 
Personal and Social History:
No excessive alcohol or tobacco use; no substance abuse; no travel; stable family and finances
 
Family History:
No relevant family history of serious diseases
