Identifiers and Chief Complaint:
72 year-old man with weakness and fatigue.
History of the Present Illness:
The patient has a long history of insulin dependent diabetes mellitus, hypertension and gout, but now has 2 months of increasing weakness, fatigue, and anorexia associated with a 60 lbs weight loss. He has had no abdominal pain, nausea, vomiting or change in bowel habits. He noted intermittent black tarry stools over the the past week but no hematemesis or hematochezia. He had had flexible sigmoidoscopy 2 months ago at an outside hospital which revealed uncomplicated diverticular disease.
Previous Medical Diagnoses, Hospitalizations, and Operations:
Hypertension, insulin dependent diabetes mellitus, gout
Medications:
Allopurinol, Furosemide, Insulin, KCl
Personal and Social History:
Drank alcohol heavily for many years but stopped 1995; no substance abuse; no travel; stable family and finances
Stopped smoking in 1997. Retired police officer and body guard.
Family History:
Father died of liver cancer, Mother had diabetes mellitus, 1 sister with breast cancer and 1 brother with gastric cancer.
