Identifiers and Chief Complaint:
A 56 year old man was referred for colonoscopy.
History of the Present Illness:
This patient was found to have a sigmoid polyp on a screening sigmoidoscopy and was referred for colonoscopy. There was no history of prior colon polyps, rectal bleeding, recent change in bowel habits or weight loss.
Previous Medical Diagnoses, Hospitalizations, and Operations:
Peripheral vascular disease
Pulmonary tuberculosis in 1989
Medications:
Aspirin 81mg PO QD
Personal and Social History:
No excessive alcohol use; no substance abuse; no travel; stable family and finances
Smoker; 30 pack years; quit 1 year ago
Family History:
No family history of colon cancer or polyps.
