Identifiers and Chief Complaint:
48 year old male with left calf pain and swelling
History of the Present Illness:
The patient has a history of ulcerative colitis diagnosed 20 years ago. He began to notice left calf pain about 2 weeks ago. It was initially mild but progressed in intensity and became associated with swelling.  His PCP obtained an ultrasound with doppler and diagnosed a deep vein thrombosis.  After hearing the diagnosis the patient was able to remember that he had a previous episode of DVT which was treated with anticoagulation.  The patient was placed on coumadin and an outpatient consultation with hematology was arranged.  He also was given an outpatient appointment with gastroenterology regarding his diagnosis of ulcerative colitis which has been in remission on sulfasalazine.

Sulfasalazine
Pyridoxine
Personal and Social History:
No excessive alcohol or tobacco use; no substance abuse; no travel; stable family and finances
Family History:
Father died at age 68 with pancreatic cancer
