Before proceding, ask yourself the follow questions:

1. What is the differential diagnosis of the patient's abdominal distension and diarrhea?

2. How do you explain the altered mental status, hemodynamic instability, acidosis, and extreme leukocytosis?
3. What further tests would you perform?

4. As further data are collected, continue to ask yourself about your leading diagnosis.
