HEENT: dry mucous membranes, otherwise normal
   
Lungs: normal
Heart: tachycardic, regular; no murmurs, rubs, gallops.
Abdomen: distended, tender to palpation with guarding, no rebound; no bowel sounds; no shifting dullness.
Rectal: brown loose stools
Neurologic: only oriented to self, not place or time; moves all extremities spontaneously but does not follow commands
