Identifiers and Chief Complaint:
63 year old man with abdominal distension and pain
History of the Present Illness:
The patient was admitted for gangrene of his left leg and subsequently had a below knee amputation. One week later, he started having diarrhea with nausea and vomiting of bilious material. Two weeks after the surgery, he was noted to have altered mental status and was hypotensive and tachycardic. This was associated with abdominal distension. He did not have any hematochezia or melena.

Diabetes Mellitus
Hypertension
Peripheral vascular disease
Abdominal aortic aneurysm repair (1998)
Medications:
Gatifloxacin
Clindamycin
Meperidine prn
Promethazine prn
Ranitidine
Benztropine
Insulin sliding scale
Personal and Social History:
Smokes 1 pack per day. No excessive alcohol use; no substance abuse.
Family History:
No relevant family history of serious diseases
