Gastroesophageal Reflux Testing

An upper GI series will evaluate for structural anomalies such as vascular loops, hiatal hernia, esophageal webs, gastrointestinal duplications, foregut cysts, and Zenker's or other diverticulae. It can also evaluate gastrointestinal tract coordination and motility. The estimation of reflux is sensitive, but specificity is low, and an upper GI may not identify clinically significant reflux.

Diagnostic Protocol
GERD
Testing
pH probe -> Treat GERD
Upper GI series -> Treat any anatomic abnormalities
Esophagoscopy -> Consider biopsy

A "milk scan" (gastroesophageal scintiscan) is a scan in which a child drinks radiolabeled formula, and then is placed in a gamma counter which demonstrates the speed of stomach emptying and if reflux occurs. Unfortunately, the results of this study often do not correlate with those of the pH probe. The study is not practical and does not quantitate GERD, therefore this test is not recommended.
