To complete the application:

1. Download it to your computer by clicking the link below.(please see more detailed mac instructions below)

2. When prompted, please choose save file

3. Open the document and fill it in

4. Save it

5. E-mail it as an attachment to sharir@bcm.tmc.edu

Macintosh Users

The membership application has been tested on Internet Explorer, Netscape and Safari, if you are having trouble please do the following:

Before clicking on the link, please hold down the "control" key, then click the link and pick the option "save link as" or "Download target link to disk", this will save the file to your computer.

Then proceed to open the saved file.

If using Netscape please use 6.2x or higher to ensure compatibility(NOT compatible with 4.7x)
