As there is no core Anesthesiology Residency program, The Methodist Hospital offers specialty rotations in cardiovascular/thoracic anesthesia, neuroanesthesia, regional anesthesia, and critical care medicine. The various rotation opportunities are listed below.
Cardiovascular/thoracic/TEE Anesthesia Service:
Residents are involved in the management of various procedures including all types of open heart surgery, thoracic surgery, major vascular surgery, transplantation, including heart, lung and kidney. They also are highly involved in the employment of intraoperative transesophageal echocardiography, with instruction provided by faculty who are experienced and certified in TEE. In addition, residents will have the opportunity to place thoracic epidural catheters for postoperative pain management on a variety of cases. Nearly all cases assigned to residents on this service are index cases, and faculty staffing is usually one-to-one
Neuroanesthesia
Is a one month rotation in one of the busiest Neurosurgery Services in the nation, with a large volume and a great variety of cases. The resident will be assigned to the most interesting and challenging cases of the day. Faculty instruction is provided by a small group of dedicated anesthesiology staff with particular interest and expertise in neuroanesthesia. Although the emphasis is clinical with plenty of experience in various types of procedures, a special effort is made to provide teaching in the operating room with discussion of neuroanesthesia related topics.
The regional anesthesia
Rotation at the Methodist Hospital is a one month rotation for upper level residents. The focus of this rotation is hands-on exposure to a great variety of regional anesthetics, including regional blocks for postoperative pain control of the upper and lower extremity, spinals, thoracic epidurals and intravenous regional blocks. All nerve blocks will be performed under direct supervision of the staff anesthesiologist. Anatomical and regional anesthesia textbooks are available on site in the block room adjacent to the operating rooms. If there are blocks scheduled at other operating room areas, every effort will be made to make this opportunity available to the block resident. Instruction will be provided by a small group of staff anesthesiologists with an interest and expertise in regional anesthesia.
The Critical Care
Rotation is conducted in the 40-bed Cardiovascular Intensive Care Unit, which is adjacent to the cardiovascular operating rooms in the Methodist DeBakey Heart Center. In order to develop the appropriate knowledge and skills for working in the ICU setting, the resident will participate in the initial assessment of patients who are admitted to the ICU during the day, performance of critical care procedures including demonstrating working knowledge of indications, contraindications, proper equipment and performance of those procedures, and participation in family discussions and conferences as appropriate in conjunction with attending physicians. In addition, there will be ongoing reassessment of the critically ill patient regarding various organ systems, psychological status, nutritional status, fluid status and any new clinical problems that require intervention. Teaching rounds and lectures will be provided daily by a diverse group of faculty, all certified in critical care medicine.
